With the “Hybrid Method” who developed by Turkish Professor, Buerger’s disease can now regress or prevent limb loss in eligible patients. With the same method, the foot of a who since 20 years Buerger patient was saved from cutting.

Mustafa Kaya, 48, was infected with Buerger Disease 20 years ago. Cardiovascular Surgery Specialist Prof. Dr. Yusuf Kalko about Buerger’s disease shared the following information.

“Buerger’s disease is a serious discomfort caused by blocking of small and medium-sized arteries and veins due to smoking.The vessels are blocked with an inflamed clot, and the blood supply to the fingers decreases.This vascular problem persists as long as smoking exists in your life.Over time, it affects your fingers, hands, feet, legs, and wounds that do not heal are formed. These deep wounds due to infection turn into gangrene over time and cause limb loss if smoking is not stopped. In more than a quarter of the patients, the feet or legs come to the cutting stage. Buerger’s disease usually manifests itself between the ages of 20 and 40. However, today, unfortunately, we see that it has reached the age of 16-17. In recent years, we have started to see this ailment, which we generally know as a male disease, in women. Buerger’s disease develops due to smoking. However, people who are exposed to intense cigarette smoke in indoor environment and who we call passive smokers are also in danger. Because this disease is actually a cigarette allergy… It is not seen in everyone, but if you have an allergic predisposition, even if you do not smoke but if you are passive smokers affects you and reveals the discomfort. To prevent or get rid of this disease, it is necessary to completely avoid smoking and prevent yourself to exposure cigarette smoke. Because if you go back to your old habit after 30 years or 50 years, your old illness will also return to you.”

Treatment method for patients with no chance of surgery: “Hybrid Methods”

Prof. Dr. Yusuf Kalko stated that with the new techniques they developed in the treatment of Buerger, they achieved very successful results in eligible patients. He said, “The most important step of treatment in Buerger patients is to stop smoking. If cigarettes are not avoided, other treatments will not help. Medical treatments do not have a definite chance of recovery. The risk of amputation (cutting of tissue and limb) is quite low in people who quit smoking. If smoking continues, the wounds do not pass, reopen, and multiple amputations may be required. In a small number of patients with this disease, the vessels are suitable for bypass surgery. But in a large number of patients, the disease does not respond to treatments and it is decided to cut the limb. We also apply the Minimal Invasive Vascular Surgery method in vascular diseases to the eligible Buerger patients. We perform a bypass by interfering with the problem area with local anesthesia and tiny incisions. Some patients may lose alittle limbs, but we can save the foot, which is very important for the patient. Another technique we have developed is Hybrid Methods. We actively use hybrid methods in carotid artery diseases and leg vessel occlusions. This technique gives us the chance to perform open surgery, angio, balloon applications and stent applications at the same time. Therefore, many patients who have not been given the chance to operate before can now benefit from this. We also started applying hybrid methods to Buerger patients. It can be applied to any Buerger patient whose main vessel is blocked. We can apply this in a patient with a blocked knee, groin, above-knee and below-knee region and no blood flow. Since the inflammation that obstructs the vessel of Buerger patients is very soft and mucus, it is very difficult to clean this vessel by standard methods. With the technique we developed, we can pull the plaque that is like a worm from 20-30 centimeters without breaking it. Just removing it is not enough. We also enter this plate from the knee with an angio instrument and we can open the vessel to the foot by making small balloons with the help of a thin wire. Then we bring the vein because we open it up and down. We stitch end to end. Think of it as connecting two pipes. And continuity is provided. Patients with very serious pain relax immediately after surgery. Most importantly, we prevent limb losses. In this way, many patients who are not given the chance to operate are given the chance to operate. Since both open and closed methods are used at the same time in our technique, the processing time varies between 1-2 hours. It is made by numbing the waist. Patients should definitely stay away from smoking after surgery and should use blood thinners for a life. We used the Hybrid Method in Mustafa Kaya. The result was very successful.
