In brief: Lymphedema is long-term swelling — usually of an arm or leg — caused by a build-up of lymph fluid when the lymphatic system cannot drain properly. It may be primary (developmental) or secondary (after surgery, lymph-node removal, radiotherapy or infection). It is different from venous swelling. What Is Lymphedema?
The lymphatic system drains excess fluid and waste from tissues. When it is damaged or underdeveloped, protein-rich lymph fluid accumulates and causes persistent swelling, most often in a limb. Without management, the swelling and skin changes tend to progress over time.
Types: Primary and Secondary
- Primary lymphedema: due to an inherited or developmental problem of the lymphatic vessels; may appear at birth or later in life.
- Secondary lymphedema: the more common form, caused by damage to the lymphatic system — for example after cancer surgery, lymph-node removal, radiotherapy, severe infection or injury.
Symptoms
- Swelling of part or all of an arm or leg, including fingers or toes
- A feeling of heaviness, tightness or reduced flexibility
- Skin changes: thickening, hardening (fibrosis) over time
- Recurrent skin infections (cellulitis)
The Stages of Lymphedema
Lymphedema is commonly described in stages (see the diagram above):
- Stage 0 (latent): lymph drainage is reduced but there is no visible swelling yet.
- Stage 1: soft swelling that improves with elevation and leaves a pit when pressed.
- Stage 2: swelling persists; the skin becomes firmer and pitting decreases.
- Stage 3: marked swelling with skin thickening and fibrosis (elephantiasis).
Diagnosis
Diagnosis is usually clinical, based on history and examination. Additional tests — such as Doppler ultrasound to rule out other causes, or specialised lymphatic imaging — may be used to confirm the diagnosis and plan treatment.
How It Differs from Other Swelling
Unlike venous swelling (which often improves overnight and is linked to varicose veins) or swelling from heart, kidney or liver problems, lymphedema is typically a firm, progressive swelling that may involve the toes/fingers and is caused by impaired lymph drainage. Correct diagnosis ensures the right treatment.
Possible Complications
- Recurrent skin infections (cellulitis)
- Progressive skin thickening and reduced mobility
- Rarely, long-standing severe lymphedema carries other risks — making early management important
Is Lymphedema Treatable?
It often cannot be fully cured, but it can be well controlled. The mainstay is complete decongestive therapy (CDT), which combines manual lymphatic drainage, compression (bandaging/garments), exercise and meticulous skin care. Early, consistent treatment keeps swelling down and prevents progression.
Living with Lymphedema and Prevention
- Wear compression garments as advised
- Daily skin care to prevent infections; treat any cut promptly
- Stay active and maintain a healthy weight
- Attend regular follow-up to adjust treatment
When to See a Doctor
See a doctor if you notice persistent swelling in a limb, a feeling of heaviness or tightness, skin changes, or recurrent infections — especially after cancer surgery or lymph-node treatment. Early assessment leads to better control.
